1. Appointment Scheduling and Cancellations
1.1 Appointment Scheduling To schedule an appointment, please contact our Practice Manager directly at office@salindahealth.com
1.2 Cancellation Policy We understand that life can be unpredictable. If you need to cancel or reschedule your appointment, we request that you provide us with at least 48 hours' notice. This allows us to offer your appointment slot to another patient who may be in need of our services.
1.3 No-Show Policy Failure to cancel or reschedule an appointment within the 48-hour window will be considered a "no-show." Repeated no-shows may result in additional fees or limitations on future appointments.
1.4 Cancellation and No-Show Fee To ensure the quality and consistency of care at Salinda Health, we have established a fee for late cancellations and missed appointments. This policy helps us manage our schedule effectively and continue providing excellent service to all our patients.
Fee Structure:
For Aetna Patients: A fee of $75 will be charged for appointments that are canceled with less than 48 hours' notice or for missed appointments.
For Out-of-Pocket Patients: The full amount of the scheduled session will be charged for appointments that are canceled with less than 48 hours' notice or for missed appointments.
We appreciate your understanding and cooperation in helping us maintain the high standards of care we strive to offer.
2. Payment Policies
2.1 Payment Due All payments for services rendered are due at the time of the appointment.
2.2 Missed Payments If a payment is missed or declined, we will make one attempt to contact you for resolution. Failure to settle outstanding balances within 30 days may result in the suspension of services until the balance is cleared.
2.3 Insurance Billing For patients using insurance, we will file claims on your behalf, but you are responsible for any co-pays, deductibles, or services not covered by your insurance plan.
2.4 Returned Checks There will be a fee for any returned checks due to insufficient funds. This fee will be added to the outstanding balance and must be settled promptly.
